National Advance Care Planning Week – Use My Health Record to capture what is important to you!

National Advance Care Planning Week is being held from 21-27 March 2022. The week is a great time to talk to your loved ones or health practitioners about your future health care wishes or preferences.

Discussions regarding what you value most when it comes to your health benefits both you and your loved ones, as it helps to ensure they can support you to receive the care you prefer when you can’t speak for yourself. Whether it’s for you or someone you love, get involved in National Advance Care Planning Week and have those important discussions.

To ensure that these discussions and preferences are known, recorded and accessible when it’s needed, we encourage you to upload a completed advance care planning document to your My Health Record. This document can then be used by your nominated and authorised representative/s or medical professionals to guide decisions regarding your health care if they were ever needed in a medical emergency.

You can add an advance care planning document to your My Health Record at any time.
