The Community Transport Organisation has joined forces with the Sydney North Health Network to launch an ‘early intervention’ program.

It's designed to help older Australians living in the Sydney North Health Network region access health appointments and associated social programs via local community transport services.

The goal is to improve the health, wellbeing, and independence of people throughout their ageing journey.

How can this program help me ?

The community transport early intervention program will provide free, accessible, and safe transport from your home to your local general practitioner, allied health appointment, wellbeing programs, or social outings.

Why is early intervention so important?

When it comes to our health, prevention is better than cure.

Regular visits to your health professional, exercise, social engagements, therapeutic treatments, and early intervention into health warning signs are the keys to staying strong, healthy, and independent as we age.

The challenge for many Australians is how to get to these appointments.

A growing number of older people are postponing health checks until symptoms are more advanced which results in complex health conditions and hospital stays.

The program is funded by Sydney North Health Network, supported by the Community Transport Organisation, and delivered by community transport providers: Easylink, Hornsby Ku-Ring-Gai Community Transport, Stryder, and Community Connect.
