Giovanni's Story

Giovanni has experienced a decline in cognitive and physical abilities and has been admitted to hospital.

Giovanni is an older man who owns a number of properties across Qld. Most of those properties are tenanted and have mortgages. 

Giovanni has experienced a decline in cognitive and physical abilities and has been admitted to hospital as living at home was becoming unviable. His daughters had been appointed as his attorneys; however, they were unwilling to accept the role. During the hospitalisation, a QCAT application was lodged and finalized, and the Public Trust and Public Guardian are both appointed to make Giovanni’s financial, accommodation and service decisions. 

Giovanni has expressed a wish to return to his home, with paid services, or alternatively, move to the local RACH. 

Giovanni was referred to the SLASSS program by his hospital discharge planner, as Giovanni was frustrated with a lack of progress.  

A number of roadblocks were encountered in Giovanni’s situation. The SLASSS social worker developed stakeholder meeting schedules to ensure there were no further missed opportunities. 

Initially, the delegate guardian and trust officer were at odds with a plan for discharge, until it was finally settled that returning home with paid services was financially unviable for Giovanni. Giovanni’s next alternative to move to the local RACH was thwarted by the RACH difficulty in engaging with PT and OPG, resulting in no offer being made to Giovanni. 

Eventually the SLASSS social worker, through many discussions with Giovanni, provided his updated preference, that he reluctantly move to aged care in the nearby region, rather than continue to languish in the hospital. 

A broker was engaged, a RACH bed was located and approved, and Giovanni was safely transferred to the RACH, within 3 weeks; some 400 days after initially being hospitalized. 

The SLASSS social worker has checked in with Giovanni, and he reports being happy and settled in his new accommodation.
