Making an Enduring Power of Attorney in your state or territory

Make an Enduring Power of Attorney with these state and territory checklists.

Last updated: 28 February 2024

Each checklist will guide you through the process of making an Enduring Power of Attorney (EPOA, also referred to as an EPA) in your jurisdiction. NB: Known as an Advance Personal Plan (APP) in NT

It will show you what to think about and connect you to relevant information.

 Each checklist includes:

  • Background information and resources to help you research and prepare to make an EPOA/APP.

  • Practical information you will need to consider before completing the EPOA form.

  • Helpful information to help you complete the EPOA/APP form.

  • Information about how to correctly sign and witness your EPOA/APP.

  • Information about registration requirements and tips on where and how to store your EPOA/APP.

  • Information to assist you should you want to change or revoke your EPOA/APP.

  • A PDF version that you can print out and keep or share with another person
